MISSION STATEMENT

VICTOIRE Health Service is a Residential Service Agency fully capable of meeting Your needs of assistance with activities of daily living. We provide Residential Services for all Clients regardless of age, sex, race, religion, color, national origin, handicap, or disability.

VICTOIRE Health Service relieves families and caregivers from having to bear all the stress of care-giving alone. We can also be contacted for respite care-giving, if a family member needs a break from caring for their loved ones everyday.

Our personnel are certified by the State of Maryland, (verified at the Maryland board of Nursing), as (CNA) Certified Nursing Assistants, and/or Medicine Aides.

We are committed to providing a quality Residential Services and meeting the special needs of each Patient in the comfort and security of their living environment.

Every member of staff of our agency undergoes a criminal background screening, to ensure that Our Agency does not send to Your Home, anyone who has been charged of abuse of any sort or that will do away with Your personal belongings.

Victoire health Services has been and is capable of providing residential Services for Senior citizens and Young adults who need assistance with activities of daily living in their home settings, in Baltimore City, Baltimore County, Harford County, and Howard County.

In addition to our excellent quality of residential Services to all our clients, We temporarily staff RN, LPN, CMA (Medicine Aides), GNA and CNA in Hospitals, Nursing Homes and Assisted living facilities.

Our main objective is not to remain stagnant, but to continue to learn to know our Client''s needs on a day-to-day basis. By so doing, we are able to continously provide excellent Residential Care to Clients.

We do not just want to be good at what we are engaged in, we want to continue to be excellent in discovering ang applying new methods of providing excellent care to all.

Patient''s Rights

All patients have the right to be notified of their rights and responsibilities before treatment begins or during the initial evaluation visit. A patient''s legal guardian or parent, in the case of a minor, may exercise these rights on behalf of the patient. If the patient is unable to read this statement of rights, it will be communicated to him/her in a language he/she can understand.

As a home care client, you have the right to-

Choose your Residential Service Agency.

Be fully informed in advance about the care and treatment to be provided by the Services.

Refuse care, to the extent permitted by law, and to be informed of the possible consequences of the decision.

Participate in the planning of care and treatment and any changes that might occur.

Voice grievances about care without reprisal.

Confidentiality of your clinical record to ensure appropriate release or review under Federal or State law.

Have your property treated with respect.

Be informed orally and in writing about all items and services to be provided, the availability and extent of coverage for those items, and costs for services for which the patient is responsible.

Be informed about costs, charges, and coverage, as well as your right to appeal any denial.

Know that you may specify advance directives through such vehicles as a living will or a durable power of attorney should you become mentally or physically unable to choose or communicate your wishes regarding care.

Be free from emotional, psychological, sexual and physical abuse and from exploitation by the home care provider.

Be free from chemical and physical restraints except as authorized in writing by a physician.

Be ensured of confidential treatment of all information contained in your personal clinical record, with your written consent required to release such information to anyone not otherwise authorized by law to receive it.

Medical information contained in your patient record shall be deemed to be your property and you have the right to a copy of such records upon request and at a reasonable cost.

Responsibilities

As a Residential Service Agency client, you have the responsibility to -

Provide a safe environment for the provision of care for the home care staff.

Keep all weapons in the home away from the work area during visits made by the staff.

Secure aggressive or menacing pets before the staff enters the home.

Repair all hazardous structural barriers.

Provide complete and accurate information to home care staff.

Inform the provider when you are unable to keep an appointment for a home care visit.

Participate in developing and following the plan of care.

Request information about anything that you do not understand, and express to your home care Services any concerns or problems regarding your home care services.

Inform the home care provider of the existence of your advance directives and any changes you make to them.
